Patients Advanced Personal Action Plan
The resource is to be filled out by the patient and family and is intended to be a communication tool that outlines how the patient wants to be cared for. It also outlines the patient’s personal journey with specific information on what in important to them, what treatment has or has not worked for them etc. It is the responsibility of the health care provider to review the advanced personal action plan with their patient, so they are aware how the patient wants to be cared for.
