Co-management Resources
Heart Failure Zones
This form can be used by patients or families who are interested in co- managing their heart failure care. Each zone instructs the patient to assess aspects of their heart condition and implement the appropriate supports and/or intervention to co-manage their care. As well, a health care professional should instruct you on the usage of the form and when and how to access the most appropriate support within your specific community. If you have any questions consult your doctor, nurse practitioner or heart function clinic nurse.
